Ensure that you clearly indicate which course you wish to attend, on what dates, and at which venue.

Payment options include credit card or cheque. The below form may be completed with credit card details and submitted directly or posted.  For posting with a cheque please print the completed registration form and make cheques payable to Emed Ultrasound.

Terms & Conditions
Upon receipt of the registration payment a confirmation letter will be sent to you with a receipt. Registration is confirmed on a first-come first-served basis.
Directions, venue details and a local accommodation list will also be forwarded to you.

Cancellation
A full registration refund will be given if cancellation is received more than 30 days prior to the scheduled programme. Cancellation after this time will result in a loss of the full course fee unless the attendee transfers to an available course. In the event the course is rescheduled or cancelled, attendees will be given the choice of transferring to another course date or receiving a full refund.
